Split Skin Grafting versus Miniature Punch Grafting in Vitiligo: A Comparative Study

Abstract

Background: Vitiligo is a common depigmentation disorder that can lead to significant cosmetic and psychological concerns. Surgical interventions, including skin grafting techniques, are often used to manage stable vitiligo when medical treatments fail. Split-skin grafting and miniature punch grafting are two commonly employed techniques. This study compares the outcomes of both methods in terms of repigmentation, donor site morbidity, and cosmetic results.

Objective: To evaluate and compare the effectiveness of split-skin grafting and miniature punch grafting in the treatment of vitiligo.

Methods: A total of 60 patients with stable vitiligo were randomly assigned to undergo either split-skin grafting or miniature punch grafting. Both groups were followed for a period of 12 months to assess repigmentation, donor site healing, and cosmetic outcomes.

Results: Split-skin grafting showed a higher rate of repigmentation (90%) compared to miniature punch grafting (75%). However, the donor site morbidity was lower in the punch grafting group. Both techniques provided satisfactory cosmetic results, but the punch grafting technique was associated with less scarring at the donor site.

Conclusion: Both split-skin grafting and miniature punch grafting are effective surgical methods for treating vitiligo. However, split-skin grafting offers better repigmentation rates, while miniature punch grafting provides a more cosmetically favorable result at the donor site.
